Journey Home is a privately, incorporated home, offering care to two persons at one time, within a home setting.
Residents at Journey Home are referred to us by one of two Certified Hospice agencies: UR Medicine Hospice or Lifetime Care Hospice.
Each prospective resident must have a terminal illness with a prognosis of three months or less. They are then interviewed by our nursing staff to make sure that Journey Home is a good match for their needs and for ours.
We do not discriminate against age, race, religion or gender. We do accept all illnesses including HIV+/AIDS.
We are unable to welcome active smokers due to our insurance limitations.
Since Journey Home opened in May 1999, we have been privileged to serve over 500 persons and their families. Ages have ranged from the late twenties all the way up to 102!
Journey Home offers care to persons who are prognosed to be at the end of their life. Our mission is to minister to the resident and their families on this most delicate and often difficult journey. We address their physical, emotional and spiritual needs, in a holistic manner, as they journey through their dying process. Comfort is maintained while allowing the body to continue with its normal and natural progression. Journey Home allows a person to live life fully until the day they die. We do not interfere with the dying process.
